Attention: Principals and School Program Planning Teams
Accessing psychology services
The service delivery model for the 2007/08 school year will
utilize a central referral process.
Services to be accessed using this model include psycho-educational assessments,
counseling and behaviour assessments/consultation. Completed referral forms, work samples and
program adaptations/IPP/BMP should be forwarded to Carol Arthurs or Natasha
Yorke-Phillip at Meadowfields
Community School
via inter-office mail or they can be dropped off at the school. The referrals will be reviewed on a weekly
basis and priorities will be determined at that time by the psychologists. You will then be contacted to obtain parental
consent for the selected student(s) or to provide additional information to
assist in determining priorities. Please
do not obtain parental consent until you have been contacted by a psychologist
indicating that the requested service will be provided. Once consent forms have been received by a
psychologist, you will be notified of the date of the
service. Consultation with a
psychologist is available to assist in determining which students you will be
referring and in determining what information to include in your referral. A school psychologist can attend program
planning team meetings upon request.
