General Information - What the Report Card Looks Like

Below is my basic report card in a table form.  By the end of the year I expect to be able to put reporting codes in all boxes.   

Reporting Codes:    A = Always  M = Most of the time  S = Some of the time  NY = Not Yet  N/A = Not Applicable at this time

Listening and Following Directions

Listens to and follows teacher's directions   Listens when other students are speaking

Social Behavior
